About The Position

The Case Management Assistant works under the direction of an RN case manager or social worker to help facilitate patient discharges requiring post-acute placement or services and identify and report barriers to discharge. The role also provides administrative support for Case Management functions.

Requirements

• At least one year of experience in a health care setting.
• Medical terminology knowledge.
• Ability to establish and maintain collaborative and effective working relationships.
• Ability to communicate effectively in oral, written, and electronic formats.
• Analytical and critical thinking abilities, with proactive decision-making and negotiation skills.
• Ability to perform specific competencies as identified.
• Self-motivated, goal-directed, and dependable.
• Computer and typing skills.

Nice To Haves

• Associate degree preferred.
• Healthcare professional experience, such as PCT, CNA, or EMT, preferred.

Responsibilities

• Assist with facilitating patient discharges.
• Identify and report barriers to discharges.
• Provide administrative support in Case Management functions.
