About The Position

Program Services Case Coordinator. This role involves providing organizational assistance and program support, including admissions procedures, direct participant care, and coordination of internal and external services. The Case Coordinator will develop individual programs to meet participant needs, optimize their potential for community integration, and ensure the provision of appropriate services. This role also involves assisting participants in understanding program goals and processes, acting as a liaison for families, organizing participant conferences, and reviewing participant reports for consistency and goal coordination. The position requires ensuring appropriate documentation and record-keeping, developing and coordinating discharge planning, and assisting with internal communication regarding case-specific issues. The Case Coordinator will attend required meetings, review monthly bills for discrepancies, coordinate utilization with the Case Manager, and organize internal medical appointments. Additionally, the role involves ensuring appropriate documentation for medical appointments, directing and coordinating healthcare planning, and communicating with external resources to ensure quality participant healthcare. Assistance in training new staff on medical care oversight is also part of the responsibilities.

Requirements

  • High School diploma or equivalent
  • Two years of direct or indirect support services to persons with disabilities in a medical and/or rehabilitation setting
  • Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines
  • Effective communication skills to manage relationships

Responsibilities

  • Provide organizational assistance and program support, including admissions procedures, direct participant care, and coordination of internal and external services.
  • Develop individual programs to meet participant needs and optimize their potential for community integration.
  • Ensure the provision of appropriate services to designated program participants.
  • Assist designated program participants in understanding overall goals and processes.
  • Act as a liaison for families, providing information and resources.
  • Organize and chair program participant conferences.
  • Review participant reports (evaluation, progress, and discharge) for consistency and coordination of goals.
  • Ensure appropriate documentation and record-keeping of designated program participants.
  • Responsible for development, coordination, implementation, and follow-up of discharge planning.
  • Assist in internal communication to team members regarding case-specific issues.
  • Attend required meetings, including clinical team meetings and supervision meetings.
  • Review monthly bills for designated program participants and identify discrepancies.
  • Coordinate utilization with the Case Manager relative to program participant’s incentive account.
  • Organize and facilitate internal medical appointments, overseeing timely completion of annual medical appointments.
  • Ensure appropriate documentation (including summaries and tracking information) for internal medical appointments.
  • Direct, develop, coordinate, implement, and follow up on healthcare planning.
  • Ensure appropriate medical follow-up for designated program participants.
  • Communicate with community resources and external persons to assure quality program participant healthcare.
  • Assist in training new staff involved with overseeing medical care.
