About The Position

Hamilton-Madison House’s Caregiver Services Program serves informal/unpaid caregivers of older adults and their loved ones in Manhattan's community districts 1-7. The program offers supportive counseling, information and assistance with benefits/entitlements, support groups, and caregiver training. It also conducts outreach and provides limited respite and supplemental assistance services. The Case Aide, under the direction of the Program Director, supports Care Managers and program operations by assisting with documentation, client follow-up, service coordination, and administrative tasks. This role is crucial for ensuring timely, accurate, and compliant service delivery, supporting caregivers, and maintaining program records.

Nice To Haves

  • Associate or bachelor’s degree preferred
  • Experience in social services, community programs, or administrative support preferred
  • Proficiency in English and at least one additional language preferred (Cantonese, Spanish, or other languages)

Responsibilities

  • Assist Care Managers in applying for Entitlements and Benefits for Caregiver and Care receiver.
  • Assist Care Managers with data entry and maintenance of client records in the NYC Aging client tracking system.
  • Upload and organize required documentation, including consent forms, assessments, and service records.
  • Review files for completeness and follow up on missing or incomplete documentation.
  • Conduct routine outreach calls to caregivers, including appointment reminders and follow-up on referrals and services.
  • Document all client contacts and follow-ups in case notes in accordance with program standards.
  • Assist with scheduling appointments, home visits, and service coordination activities.
  • Provide basic information and assistance to caregivers regarding available services and resources.
  • Support Care Managers with coordination of respite and supplemental services, including communication with vendors and providers.
  • Maintain updated resource lists and referral information.
  • Assist with quality assurance activities.
  • Support program activities such as caregiver surveys, outreach follow-ups, and data collection.
  • Work with program staff to ensure services are delivered in a timely and organized manner.
  • Other duties as determined by Director and required by funding sources.
