About The Position

Charis Hearts, Inc. is a statewide homecare provider through the TennCare CHOICES program. Caregivers provide non-medical in-home and/or community care based on each client’s Client Service Plan. The role involves supporting client well-being, following service plans and agency procedures, maintaining accurate and confidential records, and reporting safety concerns. Duties may vary according to the needs of each client. The posting lists a salary description of $16.50.

Requirements

• Must be at least 18 years old.
• Must have a valid State ID or driver’s license.
• Strong oral and written communication skills.
• Reliable transportation to and from client locations.
• Compassionate and caring nature with a genuine desire to help others.
• Honesty and integrity in all interactions.
• Willingness to attend training provided by referring agencies and/or the Tennessee Department of Mental Health & Developmental Disabilities (MHDD).
• The client prefers a male caregiver.

Responsibilities

• Perform client wellness checks to ensure their well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on Activities of Daily Living (ADLs) in the client’s home.
• Provide non-medical support to agency service recipients.
• Follow all rules, policies, and procedures related to service delivery.
• Submit required documentation accurately and on time.
• Adhere to each recipient’s individual service plan.
• Maintain confidentiality of client records and information unless authorized by written consent.
• Immediately report incidents of injury involving the service recipient or caregiver to a supervisor.
• Report suspected abuse or neglect to the appropriate authorities and a supervisor.
