About The Position

  • The role may require lifting or assisting with up to 50 pounds.

Nice To Haves

  • Experience in memory care, assisted living, skilled nursing, home care, or healthcare preferred
  • Experience working with residents with dementia or cognitive impairment preferred
  • CNA/GNA certification welcomed
  • Strong communication and teamwork skills

Responsibilities

  • Provide hands-on ADL and emotional support for residents living with dementia and other cognitive impairments.
  • Assist with toileting, repositioning, hygiene, continence care, mobility, transfers, showering, eating, and other activities of daily living
  • Monitor and respond to changes in condition, behavior, comfort, gait, sleep patterns, or alertness
  • Respond promptly, calmly, and reassuringly to resident needs
  • Help prevent falls and reduce confusion or agitation
  • Support residents with reassurance, redirection, companionship, and a calming presence
  • Maintain a clean, safe, and organized environment
  • Communicate observations clearly to leadership, the nurse on duty, and the incoming shift
  • Document care accurately and thoroughly
  • Work collaboratively with nurses and caregivers as part of an integrated care team
