About The Position

- The role may require lifting or assisting with up to 50 pounds.
Nice To Haves
- Experience in memory care, assisted living, skilled nursing, home care, or healthcare preferred
- Experience working with residents with dementia or cognitive impairment preferred
- CNA/GNA certification welcomed
- Strong communication and teamwork skills
Responsibilities
- Provide hands-on ADL and emotional support for residents living with dementia and other cognitive impairments.
- Assist with toileting, repositioning, hygiene, continence care, mobility, transfers, showering, eating, and other activities of daily living
- Monitor and respond to changes in condition, behavior, comfort, gait, sleep patterns, or alertness
- Respond promptly, calmly, and reassuringly to resident needs
- Help prevent falls and reduce confusion or agitation
- Support residents with reassurance, redirection, companionship, and a calming presence
- Maintain a clean, safe, and organized environment
- Communicate observations clearly to leadership, the nurse on duty, and the incoming shift
- Document care accurately and thoroughly
- Work collaboratively with nurses and caregivers as part of an integrated care team
