About The Position

The Care Transition Specialist II collaborates to enhance the delivery of patient care services along the continuum of care. This role meets patients' needs efficiently and expeditiously by continuously improving the patient experience, helping ensure institutional standards of high-quality patient care, reducing costs, and ensuring reimbursement. The position involves researching resources and programs for patients and caregivers, providing assistance for disabled and elderly assistance programs, coordinating legal, financial, and insurance resources, and connecting patients with durable medical equipment contractors and home safety equipment resources. It also supports systems for patients and caregivers experiencing grief or bereavement, provides health education materials, and assists the Care Manager and Social Worker with follow-up patient support calls.

Requirements

  • Bachelor's Degree in Business or a related field of study.
  • Five years of experience, of which two must be related to healthcare.
  • 3-5 years of experience required.
  • Assessment and problem-solving skills.
  • Strong interpersonal skills.
  • Strong Customer service skills.
  • Ability to work independently with minimal supervision.
  • Goal-oriented and accountable in a complex medical environment.
  • Demonstrated organizational and time management skills.
  • Strong oral and written communication skills.
  • Good computer experience.

  • Experience navigating Insurance Networks is desired.

Responsibilities

  • Research resources and programs available for patients and their caregivers.
  • Provide assistance for disabled and elderly assistance programs.
  • Coordinate legal, financial, and insurance resources.

  • Support systems for patients and caregivers experiencing grief or bereavement.
  • Provide health education materials, including a hard-copy and electronic storage system that is easily accessible to program staff.
