Requirements

  • Associates degree from an accredited college or university required.
  • Two years of experience working with individuals with disabilities required.
  • Experience working with transitioning students and/or young adults with disabilities, or other relevant, transferrable work experience, required.
  • Pass and maintain Department of Human Services background clearance is required.
  • Valid driver’s license and/or access to reliable transportation to perform work-related travel required.
  • Eligible drivers must have a good motor vehicle record (MVR).
  • Ability to follow a curriculum to train and teach in a group setting in an engaging and informative manner and customize activities as needed.
  • Must have knowledge of employment transition, supported employment, and job training issues for persons with disabilities.
  • Proficiency in Microsoft Office skills, with the ability to become familiar with corporate specific programs and software.

Nice To Haves

  • Relevant degrees may include Special Education, Psychology, Social Work, Sociology, Human Services, Rehabilitation Counseling preferred, or in lieu of degree a minimum (4) years of relevant experience.
  • Prior experience public speaking, training, or teaching on a one-to-one basis is preferred.
  • Prior experience in developing partnerships with employers and/or postsecondary educational institutions is preferred.
  • A high-energy, dynamic self-starter and leader with excellent communication and interpersonal skills who is able and eager to provide broad support to high school students with a variety of disabilities.
  • A well-organized and self-directed team player.
  • An individual who can relate to people at all levels of an organization.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Responsibilities

  • Assess transitioning student’s strengths, barriers, skills and interests in all five areas of the Transition (1) Job Exploration Counseling; 2) Workplace Readiness Skill Building; 3) Work-based Learning Experiences; 4) Postsecondary Educational Counseling; and 5) Self-Advocacy Skill Building.
  • Plan, prepare and deliver instructional activities and specialized curriculum to students to facilitate the transition from high school.
  • Help students become familiar with job application forms, resumes, soft skills training, grooming and hygiene, anticipated employer expectations and general interview procedures.
  • Provide transportation training, place students in summer jobs and/ Work Based Learning Experiences, and other supports necessary for the student to obtain or maintain employment.
  • Provide job coaching supports and training for participants with disabilities so they may be successful within their employment environment.
  • Seek regular formal and informal feedback from participants and their support team members regarding pre-employment transition services provided to learn satisfaction and to update curriculum topics and activities as needed.
  • Develop and maintain positive relationships with partner agencies, school personnel, businesses, educational institutions, other departments and/or vendors to enhance menu of services, continuum of care, and the number of employment transition opportunities.
  • Provide general case management, maintaining excellent communication with participants, family members, funding and referring agencies, schools, and other partners.
  • Maintain accurate and complete records of the student’s progress and development and overall completion of curriculum, pre and post assessments, and outcomes.
  • Complete accurate and timely monthly billing reports, participant reports, case notes, and other supporting documentation.
  • Provide consultation, guidance, and disability awareness training to employers and educational program representatives to foster positive partnerships.
  • Communicate regularly with both internal and external Directors, Supervisors, Rehabilitation Counselors, and/or any other relevant individuals regarding all pre-employment transition activities and progress of participant with career and education exploration, building of work readiness and self-advocacy skills, and work-based learning experience participation.
  • Perform other responsibilities as assigned.
