This is a Full-Time Care Transition Liaison - LPN position based at Baylor Saint Luke’s Hospital Houston, TX. The role involves collaborating with healthcare professionals, identifying patient needs, and facilitating seamless transitions to home health care. Requirements

  • Graduate of an accredited school of nursing, current LPN license to practice in the state serviced
  • Minimum of (2) years of healthcare experience, preferably in home health or sales/marketing.
  • Strong communication, assessment, and decision-making skills.
  • Ability to build relationships, educate, and promote services effectively.
  • Committed to professional development and improving own skills.

Responsibilities

  • Collaborate with hospitals, nursing homes, and home health personnel to ensure quality care.
  • Meet or exceed established monthly admission target(s) as provided by their supervisor and determined by the branch monthly budget.
  • Coordinate patient referrals, evaluate eligibility for services, and assist in arranging home health care.
  • Analyze patient needs and educate caregivers about home health options, Medicare/Medicaid guidelines, and insurance benefits.
  • Develop relationships with healthcare providers to promote home health services and best practices.
  • Participate in provider education and support hospital staff as a home health resource.
  • Conduct trend analysis and identify opportunities for enhancing home health service outcomes.
  • Contribute to Quality Assurance Performance Improvement (QAPI) initiatives.
