About The Position

The Care Transition Coordinator is an experienced field marketer responsible for company growth. The role develops the company’s Home Health and Home Care presence within supportive living and home- and community-based residences. It generates new referrals by building relationships with area hospitals, healthcare systems, case managers, and other community resources. The coordinator is responsible for the referral process, including identifying home health needs, verifying benefits, and coordinating care. The role represents the agency professionally and prepares weekly reports on marketing activities.

PurposeCare provides home care and home health services in the Midwest. The company describes its services as coordinated home-based care intended to help clients remain healthy and safe at home.

Requirements

• At least 18 years old.
• One year of Home Health/Home Care, marketing, or case management experience, or a combination of these.
• Valid driver's license.
• Excellent oral, written, and active listening skills.
• Ability to work independently and maintain confidentiality.
• Sound judgment and good decision-making skills.
• Integrity and fair-mindedness consistent with company standards, practices, policies, and procedures.
• Ability to sit at a desk and listen effectively for long periods on the telephone.
• Professional appearance and demeanor.
• Ability to use office equipment.
• Patient and friendly manner.
• Proficient computer skills.
• Ability to work evenings or weekends as required.
• Must pass a criminal background check and drug screen.

Responsibilities

• Participate in short- and long-range planning to achieve company goals and objectives.
• Manage the complete referral process, including identifying home health needs, verifying benefits, and coordinating care.
• Cultivate new and existing relationships that contribute to company growth.
• Represent the agency in a professional, prepared, and caring manner.
• Prepare weekly reports on marketing activities.
• Initiate and expand the referral network.
• Comply with company compliance requirements and HIPAA regulations.
• Perform other duties as assigned.
• Uphold policies and procedures established by Purpose Home Health.
• Organize and prioritize work, meet assigned deadlines, and establish good working relationships internally and externally.
