The Care Team Manager will operate assigned homes in a manner that ensures company goals are met, including budgets and clinical effectiveness. This role involves supporting and supervising Direct Support Professionals (DSPs) to ensure they have the necessary training and skills for high-quality care. The manager is responsible for maintaining the assigned homes, ensuring all systems, facilities, and furnishings are in good working order, and assigning repairs and maintenance duties to staff when possible. They will also collaborate with their direct supervisor on contracting for approved services and evaluating contractor performance. In terms of clinical responsibilities, the Care Team Manager will work with the direct supervisor and clinical team to ensure proper clinical actions are taken to maintain or improve the health, safety, and stability of individuals. They will also oversee individuals' activities and programs, identifying ways to enrich their social, emotional, mental, and physical needs, and increase their home involvement and independent living skills. Regular communication with internal and external clinical, medical, and regulatory agencies is also a key part of this role. The manager will attend and ensure DSPs attend individual functions as scheduled and maintain regular individual interaction in accordance with their person-centered plans.
