Care Services Representative - FT Evening Shift

About The Position

This is an onsite, 40-hour-per-week position, with shifts scheduled between 12:00 p.m. and 10:30 p.m. Team members participate in an every-third-weekend rotation and work one to two holidays per year on a rotating basis. In this role, you will support patients, families, referral sources, and care teams throughout Ohio's Hospice service areas. The ideal candidate is compassionate, service-focused, and comfortable working in a fast-paced environment.

Requirements

High school diploma or GED required.
Strong computer and data-entry skills.
Excellent communication and customer-service abilities.
Valid driver's license and a safe driving record.
Proof of state-required automobile insurance.
Ability to travel in varying weather conditions when necessary.
Ability to successfully complete a criminal background check and 10-panel drug screening.

Nice To Haves

At least two years of customer-service experience preferred.

Responsibilities

Respond to incoming calls and inquiries with professionalism, empathy, and excellent customer service.
Collect and document accurate patient and referral information.
Explain hospice services clearly and respond to questions from patients, families, providers, and community partners.
Maintain complete, accurate, and timely records of all interactions.
Collaborate with team members and support other departments as needed.
Participate in quality-improvement initiatives that strengthen patient and family care.
Assist with onboarding and training new team members when assigned.
