About The Position

The Care Partner provides compassionate, person-centered care that promotes each resident's dignity, independence, safety, and overall well-being. Working under the direction of the Resident Care Director or designee, this role assists residents with activities of daily living, encourages meaningful engagement, and creates an environment where residents feel respected, valued, and at home. As a trusted member of the care team, the Care Partner develops positive relationships with residents, families, and coworkers by delivering exceptional service with empathy, patience, and professionalism. Through attentive observation, effective communication, and commitment to hospitality, the Care Partner recognizes changes in residents' physical, emotional, or cognitive well-being and promptly communicates concerns to the appropriate clinical team member. This position plays a vital role in supporting residents in Memory Care by providing individualized care, promoting purposeful living, encouraging resident choice, and fostering meaningful connections that enhance quality of life. Requirements

  • High School Diploma or GED preferred
  • Previous caregiving, hospitality, customer service, or healthcare experience preferred
  • CPR certification (or ability to obtain within 30 days)
  • Ability to complete all required state caregiver training
  • Successful completion of dementia care training
  • Ability to communicate effectively verbally and in writing
  • Ability to use electronic documentation systems
  • Ability to work flexible schedules, including weekends and holidays
  • Demonstrate compassion, patience, professionalism and emotional maturity
  • Excellent interpersonal and customer service skills
  • Reliable attendance and punctuality
  • Maintain confidentiality of verbal and written information pertaining to residents, associates, and community operations
  • Consistently performs all duties in accordance with the service and performance standards established by Parc Communities’ Gold Standards.
  • Neat appearance, adherence to dress code and good personal hygiene are expected
  • Ability and desire to work as a team member and provide services to residents

Responsibilities

  • Provide compassionate, respectful, and individualized assistance with activities of daily living, including personal hygiene, dressing, grooming, toileting, bathing, mobility, transfers, positioning, dining, hydration, and continence care.
  • Promote each resident's dignity, independence, comfort, and personal preferences while encouraging participation in daily activities whenever possible.
  • Assist residents with mobility throughout the community, including escorting them safely to meals, activities, appointments, and other destinations.
  • Maintain residents' personal living spaces by assisting with bed making, linen changes, light laundry, and creating a clean, safe, comfortable, and welcoming environment.
  • Deliver care with kindness, patience, empathy, and professionalism, recognizing that every interaction is an opportunity to enhance a resident's quality of life.
  • Follows the resident’s individual service plans as directed. Follows the daily service plan developed for each resident
  • Observe residents for signs and symptoms of illness, sleep changes, infection, or communicable disease, and promptly report concerns to the appropriate supervisor or licensed nurse.
  • Adhere to all infection prevention and control policies, procedures, and applicable federal, state, and local regulations to help maintain a safe and healthy environment for residents, families, visitors, and associates.
  • Follows written instructions and requests assistance and further instructions with new or unfamiliar situations.
  • Work collaboratively with Nursing, Housekeeping, Maintenance and Activities.
  • Exhibits a caring and respectful attitude toward the resident and the resident’s property
  • Uphold the confidentiality of residents and coworkers, HIPAA
  • Attend bi-weekly in-service meetings and mandatory meetings
  • Adheres to all community and company policies and performs any other duties as assigned
  • Demonstrates compassion, patience, empathy, and emotional resilience when caring for residents living with dementia.
  • Maintains composure and professionalism during challenging resident behaviors or emotionally charged situations.
  • Properly use, remove, and dispose of Personal Protective Equipment (PPE), including gloves, gowns, masks, and eye protection, as required by resident care needs and infection control guidelines.
  • Anticipate resident needs and provide prompt service recovery.
  • Maintain a clean, organized, and sanitary resident environment by following established cleaning, disinfecting, and equipment handling procedures.
  • Safely handle, transport, and dispose of soiled linens, biohazardous materials, and regulated medical waste in accordance with established policies.
  • Clean and disinfect resident care equipment and high-touch surfaces between uses, following community procedures and manufacturer recommendations.
  • Completes all required and mandatory monthly training related to dementia care, Alzheimer’s disease, and memory-support best practices within established timeframes.
