About The Position

A Care Navigator supports individuals who need help navigating complex social service systems and accessing appropriate resources. The role works with clients facing challenges such as mental health issues, substance abuse, and other barriers to independent living. The Care Navigator assesses individual needs, develops tailored plans, coordinates services, and advocates for clients to ensure they receive comprehensive support and can achieve their goals. The position involves working in the community and coordinating with clients, families, caregivers, service providers, community organizations, healthcare providers, and government agencies. Requirements

Associate degree or 2–5 years of experience in a related field.
Valid New York State driver’s license with a driving record acceptable to BHSN’s insurance carrier.
Reliable transportation for required travel.
Ability to work a flexible schedule to accommodate families’ needs.

Bachelor’s degree preferred.

Responsibilities

Conduct thorough assessments to identify clients’ strengths, needs, preferences, and barriers to independent living.
Gather information from clients, families, caregivers, and other service providers to understand each client’s situation.
Coordinate services and resources by liaising with community-based organizations, healthcare providers, government agencies, and other stakeholders.
Facilitate referrals to external agencies or specialists for medical, behavioral health, housing, employment, education, and other supportive services as needed.
Perform other job duties as assigned.
