About The Position

Greater Good Health is hiring a Care Navigator & Medical Assistant to support older adults in access-starved communities through a value-based, outcomes-focused care model. The role coordinates patient engagement and follow-through with assigned Nurse Practitioners, helping patients access preventative care and maintain continuity across care settings. Applicants must be comfortable working 7 a.m.–4 p.m. MST. The position is primarily home-office based, with a future clinic opening expected to include standard Medical Assistant duties such as rooming patients and taking vitals; the clinic opening timing is undetermined. The Care Navigator conducts patient outreach and scheduling for home, telehealth, and in-clinic Annual Wellness Visits (AWVs), responds to inbound patient calls, encourages preventative screenings, and helps address barriers to care. The position coordinates care with Nurse Practitioners, manages schedules and home-visit routes, obtains and reconciles outside medical records, supports post-discharge follow-up, and tracks completion of provider orders and referrals. It also supports quality-gap closure, accurate documentation, privacy compliance, interdisciplinary collaboration, and program development. Success is measured by outreach volume and quality, AWV and preventative-care scheduling, post-discharge follow-up, retrieval of outside records, completion of provider orders, strong working relationships, and complete, timely documentation. The role involves approximately eight hours per day seated at a home-office desk using a computer and keyboard, with two 10-minute breaks and a one-hour meal break.
