About The Position

The Care Navigator Acute role is part of the Utilization Management department. The position requires completion of an accredited nursing educational program and a current registered nursing license. A compact license is required to manage patients in a facility outside the applicable state. The role requires at least three years of recent experience in acute care, home health, case management, discharge planning, or care management. The position is full time, with 40 hours per week and 8-hour or 10-hour shifts. The listed pay range is $41.10–$61.65.

Requirements

• Completion of an accredited nursing educational program.
• Current registered nursing license.
• A compact license is required to manage patients in a facility outside the applicable state.
• At least three years of recent experience in acute care, home health, case management, discharge planning, or care management.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Experience as a Care Manager preferred.
• Experience with IT solutions such as electronic health records, learning management systems, or disease/care management systems is a plus.
