•$26 - $31•Onsite

About The Position

Paragon Behavioral Health Connections is seeking a Bachelor-Level Care Manager to provide intensive, community-based care management within its Community Access & Partnership (CAP) Program. The program serves adults with Serious Mental Illness (SMI) and Severe and Persistent Mental Illness (SPMI) whose psychiatric, functional, and social needs make traditional outpatient services difficult to access. Through assertive community outreach and a multidisciplinary team approach, the Care Manager provides care coordination, resource navigation, housing support, community linkage, benefits assistance, and skill development in clients’ homes and natural environments. The role supports clients in achieving psychiatric stability, maintaining housing, reducing crisis utilization, improving independent living skills, and transitioning to less intensive care. It involves extensive field-based work and community outreach throughout the Denver Metro area and surrounding communities, with collaboration among internal and external providers. The position is full-time and non-exempt, with a salary of $26.44–$31.25 per hour. It reports to the CAP Team Lead / Program Supervisor.
