About The Position

New Mexico Solutions is a Certified Community Behavioral Health Clinic (CCBHC) dedicated to expanding access to high-quality, integrated care. We are seeking a mission-driven Care Manager to join our Crisis Services team in Santa Fe, NM. This role involves providing direct crisis intervention, case management, and support to individuals experiencing mental health and substance use issues. You will be an integral part of a multidisciplinary team, contributing to clinical excellence and making a meaningful impact in the community.

Requirements

  • High School Diploma or GED
  • Minimum of 2 years of relevant experience working with the target population.
  • Demonstrated ability to manage time to meet all deadlines.
  • Demonstrated ability to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately.
  • Good verbal and written skills.

Responsibilities

  • Participate as an active treatment team member providing crisis services, modeling effective crisis services, and assisting other staff in learning to provide crisis services to individuals with mental health and substance use issues.
  • Engage and develop rapport with individuals in crisis situations presenting with a variety of symptoms/presentations.
  • Recognize and assess client risk of danger to self or others and provide appropriate responses to the situation.
  • De-escalate immediate crisis, assess client needs, and develop treatment plans to address those needs.
  • Develop safety and after care plans to reduce future crises.
  • Coordinate and collaborate with community resources to meet client needs.
  • Participate in on-call coverage and respond to on-call incidents in a manner that meets contract requirements and timelines.
  • Provide training, modeling, and coaching related to case management and crisis support to new staff members.
  • Support supervisors in coordinating activities of case managers to ensure effective coverage and service delivery for clients.
  • Act as agency contact for community partners (when assigned) to promote effective coordination of care.
  • Serve individuals with mental health and substance use issues as part of a multidisciplinary team approach to treatment and recovery.
  • Provide comprehensive outreach, engagement, and follow-up to assigned individuals, including contact attempts via phone, in-person, or in the home.
  • Provide culturally appropriate, person-centered, and culturally relevant services.
  • Maintain a non-judgmental, open, and welcoming approach to diverse populations.
  • Document all services provided within expected time frames, meeting service definitions, regulatory standards, and agency expectations.
  • Consistently meet established productivity standards through excellent organizational skills, effective time management, and strong engagement skills.
  • Participate in and complete all required trainings accurately and timely.
  • Identify and request training or skill enhancement opportunities.
  • Engage in cross-training activities with other team members.
  • Attend and participate in required meetings and supervision sessions and implement supervisory feedback in work performance.
