Care Manager LCSW: The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Responsibilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management LCSW is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral or legal barriers to accessing necessary resources and care. Care Manager MSW: The Inpatient Master of Social Work (MSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management MSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access.

Requirements

  • Master's Degree in Social Work from an Accredited Council on Social Work Education program
  • Washington Clinical Independent Social Worker License (for LCSW)
  • Washington Clinical Independent Social Worker Associate License (for MSW)
  • 2 years SW experience in any healthcare setting
  • 1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program

Nice To Haves

  • Basic understanding of acute medical surgical healthcare terminology
  • Prior case management experience
  • ACM/CCM certification (for MSW)
