Care Coordinator MSW
About The Position
The Care Coordinator MSW will provide comprehensive direct patient continuum of care and case management services. This role involves coordination and referral to health and social welfare resources, as well as discharge planning in conjunction with the Continuum Care Manager or Care Navigator. The Social Worker will function as an integral member of a collaborative and interdisciplinary team, supporting nursing services and other clinical and non-clinical ancillary services to ensure the continuum of care for patients and maintain the quality of service delivery. This position will also serve as a patient advocate.
Requirements
- LSW or LCSW license required
- Master’s Degree from an accredited school of social work required
- CPR BLS Certification required-American Heart Association (AHA)
- Two years’ post-graduate experience preferred
- healthcare and/or mental health experience preferred
- Experience with Electronic Medical Record (EMR) platform preferred
- Experience with MS Office
