About The Position

Cheer Home Care is hiring a Care Coordinator in San Diego. This role is for individuals looking to make a positive impact in their community and who enjoy spending time with older adults. The Care Coordinator is responsible for creating unique care plans, managing client care, and providing constant feedback to clients, client representatives, and caregivers. The successful candidate will work closely with the office team and must possess excellent communication and problem-solving skills.

Requirements

• Upbeat Personality
• Strong Computer Skills
• Strong Organizational & Communication Skills
• Pass Live Scan Fingerprint Clearance (LIC 9163)
• Valid driver’s license and proof of insurance required
• Cell phone with Internet Access
• Proof of Negative TB results
• Willing to Work One Weekend a Month as Back-Up On Call

Nice To Haves

• Master in Social Work, Nursing or Relevant Field, preferred
• Industry Experience, preferred
