About The Position

The YMCA Youth & Family Services is looking for a self-motivated and dynamic professional to join our Y family. About the Job The Care Manager conducts youth-centered service meetings to determine client’s strengths and needs and then what services are needed to address those needs. Provides general client services, group activies, programming, and case management in a Montgomery County Public School.

Requirements

  • Bachelor’s degree in psychology, social work, sociology, criminal justice, family science or other human service-related field.
  • 1-3 years’ experience developing and administering teen centered programming
  • Experience in working with diverse populations; and skilled at collaborating with multiple agencies
  • Relationship building (youth, parents, community, etc.)
  • Clear, effective written and verbal communication

Nice To Haves

  • Bi-lingual in English and Spanish preferred

Responsibilities

  • Accepts referrals for social services and completes initial intakes
  • Consults with county/community/agencies to identify and develop potential resources for client assistance
  • Designs intervention strategy youth and/or youth’s family to match appropriate community resource to the youth and family’s needs and makes those connections
  • Provides case management and monitoring services to youth/families
  • Creates and executes student engagement activities for teens including weekly groups, workshops and activities
  • Provides on-going social services (appropriate assistance during crises, may provide supportive counseling, food, clothing, housing, employment, transportation, financial assistance)
  • Performs accurate recordkeeping including intakes, developing, reviewing and updating the Life works assessment, contact notes and electronic database entries
