About The Position

The Care Managers service those who are enrolled in Medicaid, have Two or more Chorionic Conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma; ages 0-21 (Childrens Program) & 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs.

Requirements

  • A Masters with one year of relevant experience, OR
  • A Bachelors of Arts or Science with two years of relevant experience,
  • A License as a Registered Nurse with two years of relevant experience,
  • Relevant experience includes care coordination, case management, social services, behavioral health, medical care navigation, or community-based services.
  • Complete Children Adolescent Needs Assessment (CANS)NY training
  • Be CANS‑NY certified annually
  • Meet exam score requirements: Care Managers: minimum 70%, Supervisors: minimum 80%
  • Applicants must reside in New York State.

Responsibilities

  • Delivers all six NYS Department of Health (NYS DOH) Health Home Core Services, including: Comprehensive Care Management, Care Coordination and Health Promotion, Comprehensive Transitional Care, Individual and Family Support, Referral to Community and Social Support Services, Use of Health Information Technology to Link Services
  • Conducting NYSDOH‑required assessments and developing compliant, person‑centered Plans of Care
  • Coordinating medical, behavioral health, substance use, and social service supports
  • Engaging members through outreach, advocacy, and ongoing follow‑up
  • Collaborating with hospitals, providers, Manage Care Organizations (MCO), and community partners
  • Maintaining accurate, timely documentation in NYSDOH‑approved systems
  • Meeting Health Home productivity, engagement, and quality benchmarks
  • Participating in case conferences, audits, supervision, and training
  • Demonstrates professionalism, maintains appropriate boundaries, and follows the agency’s Code of Conduct
  • Promotes a positive, respectful, and productive work environment
  • Upholds the agency’s mission and vision in all interactions with staff and families served
