About The Position

This role is a contract opportunity for a Care Manager in Indiana, focusing on supporting patients and families within the community.

Requirements

  • 2+ years of experience working with frail or elderly disabled adults and caregivers.
  • Experience with data input, case review/case note entry.
  • Working knowledge of ADL/ IADL assessments.
  • Valid driver’s license with an insured vehicle for travel within the state.
  • Excellent verbal and written communication skills.
  • Excellent organizational and time management skills.
  • Strong critical thinking and problem-solving skills.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook).
  • Excellent training skills.
  • Team-player mentality.

Nice To Haves

  • Previous field experience in a medical or case management environment.
  • Salesforce experience is a plus.
  • Hands-on experience in home care.

Responsibilities

  • Perform orientations for new patients.
  • Conduct assessments of new patients joining the program.
  • Train caregivers to ensure optimal service delivery.
  • Evaluate the home care needs of clients.
  • Assess the environment for suitability.
  • Determine the fitness of caregivers to provide care.
  • Conduct home orientations, including paperwork.
  • Perform initial assessments during orientations.
  • Conduct periodic supervisory visits for ongoing cases.
  • Train caregivers and patients as required.
  • Interact with the patient's care manager as needed.
  • Engage in interdisciplinary meetings with care managers, caregivers, and patients.
  • Collaborate in care planning and communicate case impressions to appropriate staff.
  • Critically assess procedures and implement improvements for case evaluation, feedback, and caregiver training.
  • Adapt to developing operations by taking on various roles.
  • Travel within the state of Indiana (mileage and expenses reimbursed).
