About The Position

Crossroads Treatment Centers is dedicated to treating patients with opioid use disorder through evidence-based medication-assisted treatment (MAT). As a Care Manager, you will connect with patients monthly to address their needs and provide necessary resources, while also building and maintaining relationships with community partners. You will work closely with a dedicated team to offer exceptional patient care and participate in community events to promote Crossroads' mission and approach to addiction treatment. Our clinics are outpatient and office-based, serving over 26,500 patients across multiple states.

Requirements

  • At minimum, High School Diploma or GED required.
  • Knowledge of compliant documentation practices

Nice To Haves

  • Experience in a fast-paced environment, servicing many clients, is preferred.
  • Experience in healthcare, addiction medicine, community health, care coordination preferred.
  • Experience with building individualized care plans is a plus.
  • Lived Addiction experience is a plus.

Responsibilities

  • Participate in community awareness/engagement events to promote Crossroads, our mission, our principles, and our approach to addiction treatment.
