About The Position

As a Care Manager RN, assess and organize patient care through treatment stages, following guidelines. Manage benefits, oversee services, and act as a case manager and trainer to achieve optimal results. Effective care management helps minimize unnecessary resource use.

This position is in Case Management in Waco, Texas, with a PRN schedule that includes weekends and weekdays.

Requirements

• Associate's degree or higher.
• At least two years of professional experience.
• Licensed Registered Nurse.
• Knowledge of care management and resource and utilization management.
• Proficiency in patient assessment and care management.
• Strong verbal and written communication skills.
• Ability to observe, assess, and document patient progress in relation to a care plan.
• Ability to help patients access community resources.
• Ability to work collaboratively with internal medical staff and external partners or organizations.
• Adaptability and ability to navigate a crisis environment.
• Discretion and respect for patient confidentiality in accordance with policies and regulations.

Responsibilities

• Conduct thorough assessments, implement care plans, and collaborate with the team to provide evidence-based, high-standard care.
• Create patient care plans focused on interventions, treatment goals, and self-management.
• Assess the health needs of patients and families and provide necessary care to meet those needs.
• Support patients with self-management through education, personal visits, and telephone interactions, encouraging adherence to care plans.
• Research resources for patient needs, coordinate programs for appropriate service delivery, and provide referrals.
• Proactively identify problems and solutions to coordinate care and provide the best possible options.
• Facilitate communication and coordination among healthcare providers and facilities.
