  • Develop and maintain strong, ongoing partnerships with care managers, case managers, and insurance plan representatives
  • Become a trusted point of contact for referral sources across the network
  • Connect authentically with a wide range of personalities and professional backgrounds
  • Research and identify potential new referral sources
  • Conduct proactive outreach to bring new partners into the fold
  • Develop creative strategies to strengthen existing relationships and grow the referral network
  • Drive consistent patient referrals through relationship-driven engagement
  • Nurture connections over time rather than treating any interaction as a one-time touch
