Eliot's Family-based Intensive Treatment (FIT) services will offer short term intensive treatment for higher acuity children, youth and families for whom more traditional outpatient services may not be effective or accessible. A FIT Team is composed of highly trained Clinicians, Care Managers and Family Partners, that will work closely with a family at least three times each week, including individual, caregiver and family sessions, skills coaching and care coordination support. The FIT Team will be trained to deliver innovative and evidence-based clinical care anchored in Values Based Transdiagnostic treatment. The ideal candidate for the Family-Based Intensive Treatment (FIT) Care Manager is a compassionate and family-focused professional with a background in child, youth and family mental health, crisis intervention, and care coordination within complex service systems. They are highly organized, and skilled in building collaborative relationships with families, providers, and community partners to deliver strengths-based, culturally responsive support.
