About The Position

Northwest Regional Council (NWRC) is seeking internal candidates to fill a Care Management Program Supervisor position. This role specifically supervises Case Managers working in MAC/TSOA and Family Caregiver Support Program, as well as Presumptive Eligibility for both TSOA and Title 19 Case Management. NWRC is a multi-service agency covering Island, San Juan, Skagit, and Whatcom counties in Washington, with offices in Bellingham and Mount Vernon. The Long-term care case management program provides older adults and adults with disabilities access to community-based long-term care services. Case managers assess client needs and authorize payment for services to help individuals maintain independence at home. This position supervises Case Managers involved in these programs.

Requirements

Master’s degree in behavioral and health services (this includes social work, clinical psychology, sociology, guidance counseling, gerontology, nursing) and two years of paid on-the-job social services experience; OR
Bachelor’s degree in behavioral or health services or a related field AND four years of paid on-the-job social service experience or two years supervisory experience.
An equivalent combination of education and experience which provides the skills, knowledge, and abilities to perform the work.
Demonstrated ability to assess client health status, develop appropriate care plans, assess skill and suitability of personal care aides, and provide clinical consultation to care management staff.
Knowledge of responsibilities and resources of federal, state and local agencies responsible for administering disability, behavioral health and aging services.
Ability to communicate effectively in both oral and written format.
Ability to work independently, with a minimum of supervision.
Ability to supervise, direct, motivate and train others.
Ability to plan, organize, prioritize and coordinate work assignments and/or projects.
Ability to work as part of a team and provide leadership and structure as appropriate.
Ability to establish and maintain effective working relationships with co-workers, providers and the general public using courtesy, tact and good judgment.
Ability to work with efficiency under frequent pressure of deadlines while maintaining a professional demeanor and to formulate solutions to problems in a timely manner.
Sensitivity to the needs and values that are present in working with diverse groups of people.
Knowledge of computer systems and software programs.
Must have a valid driver's license, a vehicle available for work-related travel, and appropriate liability insurance.

Nice To Haves

Experience working with community-based programs for seniors and people with disabilities preferred.
Skill and/or fluency in speaking other languages is desired.

Responsibilities

Coordinating and overseeing the NWRC Care Management Programs.
Providing direct supervision, coaching, leadership, and training to staff, emphasizing clients’ preferred choices.
Promoting collaboration among NWRC team members.
Maintaining relationships with community partners and key personnel at the Home and Community Living Administration.
Fostering a culture of caring, knowledge sharing, and empowerment to encourage staff independence.
Assisting the team by evaluating suitable in-home long-term care programs that align with clients' preferred living arrangements and desired services.
Actively engaging in own education regarding various programs, databases, and payment systems to support staff.
Delivering individual, team, and regional team trainings.
Developing internal policies and procedures and editing them as necessary.
Collaborating with other members of the case management leadership team to resolve departmental issues, identify necessary staff training, and workforce planning.
