Care Management Professional (4686)

ncgCARE•Fayetteville, NC

About The Position

Carolina Outreach is seeking a Care Management Professional to proactively engage high-risk members, particularly those with behavioral health challenges, and support their whole-person health. The role assesses, evaluates, and coordinates members’ physical, behavioral, and support needs, identifies barriers to effective care, and helps resolve them. Care Managers develop and update Individualized Service Plans (ISPs), support medication management, and provide self-management tools beyond the counseling and guidance members receive from providers. The Care Manager coordinates communication among members and their providers, including doctors, nurses, and specialists, to bring provider directives together into a seamless care plan. The role requires frequent inbound and outbound calls, assertive engagement with hard-to-reach members, rapport-building with people with complex needs, and work in a performance-driven environment with weekly and monthly productivity standards and quality metrics. The organization describes its approach as supporting clinicians, providing flexibility to shape schedules, maintaining steady caseloads, and offering opportunities for growth.
