About The Position

Carolina Outreach is hiring a Care Management Professional to support high-risk members, particularly people with behavioral health challenges, through personalized, whole-person care. The role assesses and coordinates members’ physical, behavioral, and support needs; identifies barriers to effective care; and helps members develop and update Individualized Service Plans (ISPs). It also includes medication management and providing self-management tools beyond the counseling and guidance members receive during provider visits. The Care Manager facilitates communication among members’ providers—including doctors, nurses, and specialists—to coordinate their care into a seamless plan. The position involves monitoring members in person and by telephone for changes in clinical symptoms, treatment side effects, or complications; helping members and families set appropriate goals; coordinating with primary care and other treating providers; and connecting members with internal or community resources and referrals. The candidate must be comfortable managing a high volume of inbound and outbound member calls, engaging hard-to-reach members, and working within established productivity and quality expectations. The role is based in Fayetteville, North Carolina, with business travel. Carolina Outreach describes its organization as providing behavioral health services and supports throughout North Carolina in a safe, respectful, equitable, and inclusive environment, with a focus on helping clients develop their strengths and quality of life.
