About The Position

Coordinates and participates in activities related to Care Management services to family members and caregivers. Supports patients and the primary care team through patient-centered outreach, care coordination, panel management, and follow-up.

Requirements

• High school diploma or equivalent.
• 1–3 years of relevant experience.

Nice To Haves

• Bachelor's or master's degree in a relevant field.
• Health home or care management experience.
• NYS driver's license.
• Bilingual Spanish.

Responsibilities

• Uses patient-centered motivational interviewing techniques to build rapport and help patients improve their health.
• Provides panel management support to the primary care team to reduce the number of patients lost to care, non-compliant with follow-up care, or disconnected from primary care.
• Participates in developing, maintaining, and adjusting individualized care plans for high-risk patients, addressing medical and social barriers to accessing care.
• Serves as a professional liaison among hospitals, primary care providers, specialists, and community resources to ensure patient-centered care coordination.
• Identifies and tracks special populations, including high-risk patients and people due for preventive or chronic care services.
• Identifies and tracks patients discharged from inpatient services or the emergency department.
• Uses team-based communication strategies to close the loop on referrals, hospital follow-ups, and outstanding items in patients' care plans.
• Performs outreach in primary care sites, homes, hospitals, and neighborhoods.
