Requirements

• Must have the desire to work with mentally ill patients ranging from young adulthood through geriatrics.

Responsibilities

• Assess patients needs, develop discharge care plans, and link patients with community resources and services to meet their identified needs.
• Assist patients with applications and referrals to government agencies.
• Coordinate appointments with community-based providers, organizations, and agencies to ensure a smooth transition to the next level of care.
• Collaborate with the patient, staff members, family members, friends, and community service personnel to develop a workable, realistic plan that motivates and engages the patient in their treatment and promotes stabilization in the community and optimal quality of life.
• Facilitate psychosocial educational groups for patients and family members referencing the signs and symptoms of various mental illnesses.
• Develop safety plans for patients and review the safety plan with the patients and their support systems prior to discharge, encouraging the patient to implement the safety plan when needed.
