A Brooklyn-based home care agency is looking for a compassionate, organized Care Coordinator to manage a caseload of 30 patients receiving home care services. This is a hands-on coordination role where you'll serve as the central point of contact for patients, families, and care teams — making sure every person on your caseload is getting the right care, at the right time, in the comfort of their own home. Requirements

• Russian language fluency required
• Strong organizational skills with the ability to manage an active caseload
• Compassionate, patient-focused, and a clear communicator
• Ability to work independently and problem-solve in a fast-moving environment

• Prior experience in home care case management or care coordination is a plus

Responsibilities

• Manage an active caseload of 30 home care patients
• Coordinate and monitor care plans to ensure patient needs are being met
• Conduct assessments and follow up regularly with patients and families
• Serve as the liaison between patients, families, caregivers, and healthcare providers
• Ensure seamless communication across all parties involved in each patient's care
• Address and resolve issues or changes in patient condition promptly
• Maintain accurate and up-to-date patient records within HDA systems
• Ensure all documentation meets regulatory and agency compliance standards
• Track authorizations, referrals, and service delivery
