About The Position

As a Care Coordinator within our VIEW program, you play a crucial role in our mission to promote mental, emotional, and physical well-being in the community. This position provides a collaborative forum to engage a variety of service partners and advocate for the needs of individuals and their families to support the family remaining together and to improve the safety and satisfaction in their lives. This role provides connection, stabilization, and support that are designed to help youth and families avoid out of home placement and facilitate warm transitions of care for their long-term needs.

Requirements

  • High School Diploma required.
  • Comprehensive knowledge of public systems and the ability to obtain and organize information for resources in various areas across Washington State.
  • Minimum one year experience working with youth and families.
  • Exceptional communication and interpersonal skills.
  • Ability to be licensed through the Washington State Department of Health as an agency-affiliated counselor.
  • Must have strong multitasking and organizational skills.
  • Ability to work in a fast-paced environment and make ethical and safe decisions.
  • Ability to manage individual stress and perform essential job functions in a safe and professional manner.
  • Must maintain a secure and reliable internet connection as this is a remote position.
  • Ability to pass a criminal background check, per organizational standards.
  • Must be 21 years or older.

Nice To Haves

  • BA in counseling or related field preferred.
  • Experience in Behavioral Health Case Management preferred.
  • Knowledge of Microsoft Software: Teams, Word, and Excel preferred.

Responsibilities

  • Understands and actively promotes VIEW program throughout WA State, identifying ways in which efforts aimed at improving outcomes for children and families can be coordinated.
  • Adheres to VIEW program timelines for completion of care coordination tasks.
  • Participates in the on-call rotation to ensure families have access to crisis services 24/7. This may include phone support and in-person response.
  • Provides direction in the development and implementation of community resource programs to achieve desired outcomes for children and families.
  • Understands and actively promotes VIEW programs throughout the county, identifying ways in which efforts aimed at improving outcomes for children and families can be coordinated.
  • Recognizes barriers that occur in a system of care and develop and implement practices necessary to sustain system of care improvements.
  • Develops long and short-term goals and objectives for the development and implementation of agencies VIEW program services across all systems of care.
  • Serves as liaison between all child and family serving agencies and the VIEW treatment team.
  • Coordinates cross-agency communication and planning as needed.
  • Maintains professionalism and unity when communicating with clients, their families, and other representatives; preserves client confidentiality at all times.
  • Adheres to organizational ethical standards, policy and procedures, mission, vision, and values.
