About The Position

PaRC (Prevention and Recovery Center), part of the Discovery Behavioral Health family, is seeking healthcare professionals to aid patients and families dealing with alcohol and substance abuse. This is a facility-based position focused on coordinating the transition of client care from inpatient to outpatient settings, including sober living, Intensive Outpatient Programs, therapists, and psychiatrists. The role involves regular client meetings to assess aftercare needs, working with the treatment team, and providing appropriate referrals while ensuring quality care through coordination with therapists, admissions coordinators, and clinical outreach. This is maintained through weekly face-to-face 30-minute sessions with clients during their residential stay, with potential for more meetings based on stay length or circumstances.

Requirements

  • Minimum of 3 years' experience in a mental health, chemical dependency setting, or healthcare.
  • Experience working with clients and documentation in a hospital and/or mental health facility.

Nice To Haves

  • A bachelor's degree in psychology, Social Work, or other related field.
  • CADC (Certified Alcohol and Drug Counselor) Certificate.

Responsibilities

  • Develop a thorough continuing care plan aligned with treatment team recommendations, business development, and client needs.
  • Communicate with outside therapists and other appropriate parties in compliance with HIPAA.
  • Address client discharge planning needs and concerns.
  • Initiate Client Discharge Plan within 24 hours of admission and complete/review it with the client prior to discharge.
  • Coordinate care within 72 hours of admission and conduct Family Contact within 48 hours of admission, documenting both in the client's chart.
  • Facilitate 1-2 aftercare groups per week as needed.
  • Meet weekly or as needed with clients.
  • Complete Discharge Planning Progress Notes for each client.
  • Document outbound referrals in the client's chart and Salesforce.
  • Maintain knowledge of client discharge dates through the assigned therapist and admissions team, accounting for potential changes due to insurance.
  • Meet with clients a minimum of 3 sessions.
  • Maintain timely scheduled initial sessions with clients within 6 days of intake.
  • Meet with clients 1 week before discharge to secure Discharge Referrals and suggest possible extensions if the aftercare plan is not suitable for success.
  • Meet with clients 1 week before discharge to secure appointments with outside providers (Psychiatrist and Therapist appointments, securing sober living beds, etc.).
  • Conduct thorough research of recommended Outpatient services (Sober Livings, therapists, psychiatrists) to ensure quality referrals, including screening recommended referrals, internet screening, phone calls, and reserving space if transitioning to sober living.
  • Build effective and lasting relationships with referents and community providers.
