About The Position

211PBTC is seeking an enthusiastic and compassionate bilingual (English/Spanish) Care Coordinator to join their 211 Help Me Grow team. This role is crucial in connecting families with the resources and support necessary for the positive growth and development of their children. The position responds to calls from parents, childcare providers, early educators, health professionals, and social service agencies regarding concerns related to child learning, developmental, emotional, and/or behavioral issues, with a special emphasis on Spanish speakers. The Care Coordinator will use approved screening tools, such as the ASQ, to conduct screenings and determine concerns related to these issues.

Requirements

Proficiency with Microsoft Windows Suite.
Ability to enter and retrieve information in databases.
Ability to establish professional relationships.
Bilingual English/Spanish.

Nice To Haves

Bachelor's degree in Social Work, Special Education, or related human services field or associate's degree in early child development from an accredited college or university.
Special education / work experience in a related field may be substituted for a degree.

Responsibilities

Determine the need for services and provide information, assessment, and referrals to community services utilizing the 211 resource database.
Complete developmental screenings and provide referrals to qualified children as needed.
Maintain accurate records of outreach activities and client interactions.
Link callers who are potentially eligible for services to the appropriate Palm Beach County or Treasure Coast County agency based on needs expressed.
Participate in health fairs and community events to increase awareness and utilization of the Help Me Grow program and 211 services.
