About The Position

The Care Coordination RN plays a critical role in helping individuals successfully navigate transitions from hospitals or treatment facilities back into their daily lives. This role involves working alongside hospitals, psychiatric facilities, providers, and community partners to ensure patients receive safe, continuous, and well-coordinated care. It offers an opportunity for a nurse who values advocacy, collaboration, and meaningful patient relationships to use their clinical expertise to directly improve mental health outcomes in the community.

Requirements

Current Registered Nurse (RN) licensure from the Kansas State Board of Nursing.
Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers.
Ability to communicate effectively with patients and families about treatment plans and care transitions.
Demonstrated proficiency with technology including computers, electronic records, and common office equipment.
Commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes.

Nice To Haves

Bachelor of Science in Nursing (BSN).
Experience in psychiatric/mental health nursing, or physician office or outpatient practice.
Experience in care coordination or case management.

Responsibilities

Help patients successfully transition from inpatient psychiatric units, hospitals, and residential programs back into community care.
Work closely with providers, nurses, and community partners to keep treatment plans aligned and effective.
Improve patient safety by reviewing medications and coordinating follow-up care.
Educate and support patients so they understand their care plan and feel confident moving forward.
Build relationships across healthcare systems that strengthen mental health care in the region.
