About The Position

The Care Coordinator supports client services and daily operations of the Kosher Food Bank by providing intake, screening, care coordination, referrals, education, monitoring, and ongoing support. The role connects clients to food resources, health services, and social supports to improve stability and well-being. This position requires the ability to assess client needs using a clinical-informed approach, identify risk factors, and support appropriate referrals and interventions. The Care Coordinator plays a central role in food bank operations, including client intake and renewals, coordination of food distribution, volunteer support, and data tracking. Responsibilities include administering food security surveys, identifying higher-risk participants, and connecting clients to appropriate services. The position requires regular interaction with clients and volunteers in a fast-paced, community-based environment and the ability to exercise sound judgment in supporting client needs and service planning. Requirements

  • Bachelor’s degree in social work, psychology, human services, or related field required
  • Strong organizational and communication skills
  • Strong computer skills and experience with data systems
  • Valid driver’s license and reliable transportation required

Nice To Haves

  • Master’s degree in social work, counseling, or a related field preferred
  • Licensed clinician (e.g. LCSW, LMHC, LMFT) preferred; candidate with a Master’s degree working toward licensure will also be considered
  • Minimum of 2 years of relevant experience in social services, behavioral health, or care coordination, preferred
  • Background or training in social work, behavioral health, public health, or a related field with experience in client assessment and care coordination, preferred
  • Experience with food banks or food-insecure populations, preferred
  • Knowledge of Kashrut and Jewish Community resources, preferred
  • Bilingual (English/Spanish), highly preferred

Responsibilities

  • Manage an assigned caseload of food bank clients and maintain ongoing contact
  • Conduct client intake, eligibility screenings, and basic needs assessments, applying a clinical-informed approach to identify risk factors and service needs.
  • Administer Food Security Surveys and track client needs
  • Identify higher-risk clients and prioritize follow-up and referrals
  • Provide referrals to internal JCS programs and external community resources
  • Track referral outcomes and maintain follow-up contact
  • Maintain accurate client records in electronic data management systems (i.e. SoxBox)
  • Coordinate client appointments and manage daily intake schedules
  • Support weekly food distribution, including coordination and client flow
  • Assist with inventory coordination and communication with program staff
  • Welcome and assist clients and visitors; respond to inquiries and phone calls
  • Assist in volunteer coordination including scheduling, training, and supervision
  • Support outreach, community engagement, and program participation efforts
  • Assist with data tracking, reporting, and program statistics
  • Support mitzvah projects, food drives, and special events
  • Participate in community events, including evenings and Sundays as needed
  • Participate in meetings, supervision, and required trainings
