The Care Coordinator supports client services and daily operations of the Kosher Food Bank by providing intake, screening, care coordination, referrals, education, monitoring, and ongoing support. The role connects clients to food resources, health services, and social supports to improve stability and well-being. This position requires the ability to assess client needs using a clinical-informed approach, identify risk factors, and support appropriate referrals and interventions. The Care Coordinator plays a central role in food bank operations, including client intake and renewals, coordination of food distribution, volunteer support, and data tracking. Responsibilities include administering food security surveys, identifying higher-risk participants, and connecting clients to appropriate services. The position requires regular interaction with clients and volunteers in a fast-paced, community-based environment and the ability to exercise sound judgment in supporting client needs and service planning.
