About The Position

Centerstone is seeking a passionate and organized Care Coordinator to support individuals on their recovery journey and improve access to coordinated behavioral health care. This role focuses on providing service coordination for individuals receiving Medication Assisted Treatment (MAT), ensuring continuity of care across behavioral, medical, and community-based services. The Care Coordinator will help clients navigate treatment, access resources, engage with providers, and achieve their recovery goals. Requirements

  • Bachelor’s degree from an accredited college or university in Social Work, Psychology, Human Development and Family Life, Criminal Justice, Counseling, Nursing, Education, or related human services field preferred
  • Experience working in behavioral health, substance use treatment, care coordination, or case management preferred
  • Knowledge of recovery-oriented care, rehabilitation methods, and community resources preferred
  • Experience working with individuals experiencing mental health and/or substance use disorders preferred
  • Current driver’s license, acceptable driving record, and current auto insurance required
  • CPR and First Aid certification within three (3) months of employment required
  • Must obtain applicable chauffeur or passenger transport licensing requirements based on state residency if transporting clients
  • Knowledge of case management principles, rehabilitation methods, and treatment coordination practices
  • Understanding of challenges experienced by individuals with mental health and/or substance use disorders
  • Familiarity with community resources and referral processes
  • Strong organizational, time management, and documentation skills
  • Excellent verbal and written communication abilities
  • Ability to build positive relationships with clients, families, providers, and community partners
  • Ability to manage crisis situations effectively and professionally
  • Ability to work independently while collaborating within interdisciplinary teams

  • Must provide evidence of freedom from infectious and contagious diseases prior to employment and annually thereafter as required
  • TB screening required for applicable direct care and residential staff
  • Annual Health Assessment required for applicable residential programs
  • Successful completion of background checks including criminal record, driving record, abuse/neglect, and fingerprint checks
  • Completion of New Hire Orientation upon hire
  • Completion of all required trainings, including Relias training, upon hire and annually thereafter
  • Completion of required annual training hours in accordance with state and program requirements
  • Completion of Behavioral Management training within required timelines
  • Completion of Direct Service Expectations training within thirty (30) days of employment
  • Knowledge of policies, procedures, and regulations applicable to assigned programs
  • Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including repetitive movements of hands, fingers, and arms for typing and documentation tasks. Sedentary work primarily involves sitting but may include occasional walking or standing for brief periods throughout the workday.

Responsibilities

  • Coordinate services for individuals receiving Medication Assisted Treatment (MAT)
  • Review assessments and discharge planning documentation to identify client needs related to employment, healthcare, education, social support, and criminal justice involvement
  • Coordinate outpatient behavioral health and substance use disorder services as needed
  • Maintain communication with referral sources, primary care providers, and treatment teams regarding client progress and care coordination
  • Request and review prior treatment records and medical correspondence as needed
  • Participate in staffing meetings to ensure continuity of care and treatment planning
  • Assist clients with referrals to community resources and services not addressed within treatment programs
  • Support scheduling of treatment services and assist with transportation coordination when appropriate
  • Develop and update individualized treatment plans in collaboration with clients and treatment teams
  • Advocate for clients and families by linking them with community-based services and resources
  • Assist with transition and discharge planning processes
  • Complete documentation and correct chart deficiencies in accordance with agency and regulatory standards
  • Provide crisis intervention support as needed
  • Maintain weekly direct service expectations and productivity requirements
  • Participate in ongoing training and professional development activities
  • Represent the organization professionally within the community and with external partners
  • Perform additional duties as assigned by leadership
