About The Position

The Care Coordinator (CC) is responsible for coordination of care for chronic health management in the clinic setting. The CC will identify patient needs, track gaps in care management, and facilitate follow up and referral with the appropriate primary and specialty care providers. The CC will utilize interdisciplinary and interagency coordination and referral to meet patient needs. The CC works with the nursing department to facilitate integration of case management with daily patient visits, the quality and informatics department. This position includes tracking and trending of chronic health issues to ensure compliance with standards of care and optimum patient health outcomes. The CC also functions as an RN staff resource.

Requirements

  • A graduate of an accredited school of nursing with current license as a Licensed Registered Nurse (RN) in the State of California.
  • Current CPR certification.
  • Minimum three (3) years’ experience in an adult acute care setting, urgent care, or family practice outpatient setting.
  • Team player that can follow a system and protocol to achieve a common goal.
  • Proficiency in use of electronic medical records.
  • Proficient use of Microsoft Office – specifically Word and Excel.
  • Ability to organize and triage.
  • Good analytical and critical thinking skills, good verbal and written skills to effectively communicate with physicians, management and other members of the healthcare team.

Responsibilities

  • Assists all patients through the healthcare system by acting as a patient advocate and coordinating chronic health care needs.
  • Participates in appropriate team meetings and quality improvement initiatives.
  • Coordinates continuity of patient care with external healthcare organizations and facilities.
  • Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
  • Facilitates patient medication management based upon standing orders and protocols.
