The incumbent of this position is responsible for providing coordination of services for patients receiving Medication Assisted Treatment. This role involves identifying issues related to employment, education, healthcare, social connectedness, and criminal justice, and coordinating outpatient services. The Care Coordinator will maintain communication with referral sources and primary care providers, request correspondence from past treatment providers, and participate in staffing to ensure continuity of care. They will also make or assist in outside referrals for issues beyond the scope of current treatment, assist in scheduling treatment, and arrange transportation. A key responsibility is the continuous and individualized development of treatment plans. The role also includes representing the agency professionally, referring clients for medical issues, linking families with community services, advocating on behalf of families, assisting with transitional planning, correcting chart deficiencies, documenting services according to KDHE/JJA Standards, providing crisis intervention, and adhering to weekly direct service expectations. Professional development is expected through obtaining 32 hours of training every 2 years, and demonstrating interest in the company's long and short-term goals.
