The incumbent of this position is responsible for providing coordination of services for patients receiving Medication Assisted Treatment. This role involves reviewing initial assessments and discharge planning, identifying issues related to employment, education, healthcare, social connectedness, and criminal justice. The Care Coordinator will coordinate outpatient services, communicate with referral sources, and participate in staffing to ensure continuity of care. They will also make or assist in outside referrals, help schedule treatment, arrange transportation, and continuously develop treatment plans in an individualized manner. The position requires representing the agency professionally, ensuring or assisting in referrals for medical issues, linking families with community services, advocating on behalf of families, and assisting clients/families in transitional planning. Additionally, the role involves correcting chart deficiencies, documenting services, providing crisis intervention, adhering to direct service expectations, and obtaining necessary training for professional development.
