About The Position

The role of the Care Coordinator is to provide administrative support to individuals and/or families seeking care at Plymouth Psych Group. This includes greeting patients, verifying their information, scheduling appointments, answering phones, directing calls appropriately, maintaining confidential medical records, and collecting payments. The Care Coordinator acts as the liaison between patients, the back office, and clinical care staff, ensuring patient requests, questions, and communications are handled effectively.

  • Understanding of maintaining confidentiality according to HIPPA standards.
  • High degree of flexibility and ability to accommodate changing needs of the department.
  • Basic computer skills, including Word and Excel.
  • Proficiency with medical terminology.
  • Ability to multi-task, prioritize, and organize.
  • Attention to detail while remaining focused in a busy environment.

Responsibilities

  • Greet all patients, verify their personal and insurance information.
  • Assist patients or family members with completion of pre-registration forms.
  • Look up accounts for no-call, no-shows to leave messages for rescheduling.
  • Coordinate last-minute appointments and verify scheduling.
  • File orders, patient demographics, requests, and charts; shred outdated information.
  • Inform patients of the costs of care.
  • Collect and post payments according to protocols; prepare and balance daily financial registers and submit all forms and fees to the billing department.
  • Answer telephone, screen callers, relay messages, and route to the appropriate department.
  • Make necessary appointments in Practice Management, coordinating physician schedules.
  • Check voicemail every 30 minutes to ensure all patient messages are attended to.
  • Confirm call-backs, reschedules, and no-show/late cancellation policy.
  • Assist with filing and additional support needs.
  • Adhere to all applicable laws and regulations.
  • Maintain confidentiality within the workplace and community.
  • Demonstrate commitment and alignment to Plymouth Psych Group's mission, vision, and values.
  • Exhibit a high degree of confidentiality, integrity, and professionalism.
  • Handle protected health information in a manner consistent with HIPAA.
  • Perform other related duties as required, subject to change at management's discretion.
  • Show up to work on time according to the established schedule.
