About The Position

At Collective Health, the Care Coordinator helps members and families navigate the healthcare system with empathy and clarity. The role partners with care managers and clinical teams to coordinate transitions and resolve benefits questions, helping ensure members receive accessible, timely, and supportive care.

This hybrid position is based out of the Lehi office, with an expectation of working in the office at least two weekdays per week. Pay is $22.12–$27.64 USD per hour, depending on factors including location, qualifications, experience, and internal equity. The role is eligible for 5,000 stock options and benefits including health insurance, a 401(k), and paid time off.

Requirements

• At least 2 years of experience in healthcare navigation, case management, community outreach, medical assisting, community health work, pharmacy technology, or an equivalent practical background.
• Professional bilingual fluency in English and Spanish.
• Working knowledge of community health resources and social determinants of health principles.
• Ability to listen actively, communicate clearly, and organize daily tasks in a collaborative team setting.
• Experience using electronic health records, digital databases, and workplace tools such as Google Workspace.

Responsibilities

• Guide members and families by phone through care options, benefits, and local community resources.
• Partner with care managers and clinical staff to coordinate timely transitions across healthcare settings.
• Research and resolve member inquiries about plan coverage, claims, and medical record requests.
• Collaborate with internal service teams and external providers to deliver coordinated, high-quality support.
• Accurately record member interactions, care activities, and community resources in electronic health record systems.
