About The Position

The Care Coordinator works within The Ohio State University Wexner Medical Center’s System-wide Addiction Services, which develops programs serving patients across emergency, inpatient, and outpatient settings. The role facilitates patients’ movement through the health care system and provides case management for patients presenting with substance use disorder and behavioral health conditions. The Care Coordinator also educates and consults with providers, patients, and families about addiction-care regulations, criteria, terminology, funding, resources, and third-party payors. The position monitors patient care, collects data to evaluate and improve behavioral health services, and collaborates with physicians, social workers, peer supporters, and other health care workers to identify patients who may benefit from the program and coordinate assessment, treatment, and follow-up. The coordinator manages a caseload of 50 or more adult patients, supports stable community treatment linkages, and helps minimize barriers to care. Work includes discharge planning, referral triage, identifying in-network providers, making and redirecting referrals, and scheduling appointments. The role maintains knowledge of local and regional behavioral health treatment, admission and referral criteria, harm-reduction services, and addiction-care trends.
