CARE COORDINATOR STIRT 3470 Baltimore - Pueblo Job Details Job Location: Pueblo, CO 81008 Salary Range: $20.25 - $20.25 Hourly Are you looking to find personal and professional fulfillment? We are currently searching for a dynamic individual to become the CARE COORDINATOR for our unit. The Care Coordinator is responsible for coordinating care for the program's patients. All positions require pre-employment drug screening and background checks.
Requirements
- Bachelor’s degree in behavioral health and related field.
- The ability to obtain a Certify Addiction Technician (CAT) certification within 18 months.
- Must have a good understanding of substance use, mental health, and treatment delivery systems.
- Advanced computer skills are required.
- CPR and First-Aid certification (initial training and renewals paid for by agency).
- Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary.
- Valid driver's license and good driving record (must provide a current unofficial DMV driving record upon hire).
- Able to communicate effectively in a multidimensional treatment program.
Responsibilities
- Responsible for the development of care planning and patient needs assessments.
- Facilitates life skills groups following the CTP policies and procedures.
- Acts as liaison, establishing networks and collaboration with hospitals, the state, community health centers, and human service agencies about patient services and needs.
- Document admissions, care management needs, and discharge planning.
- Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake.
- Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators.
- Assists treatment team with patient intake, transfer, and discharge assessments.
- Collaborating with the facility treatment team (medical, psychiatric, and SUD) in identifying patients’ needs.
- From available resources, assist patients in meeting their basic needs (i.e., clothing, medical, judicial, religious, etc.).
