Under the supervision of the Program Director, the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized services including orientation, transportation, doctor visits, and schedules. This role ensures programs and services are implemented at the highest standards and clients receive the highest level of care. The Care Coordinator is responsible for client supervision and ensuring they attend designated groups throughout programming. They will also facilitate assigned groups and activities, conduct client orientation, and ensure compliance with Joint Commission Standards. Documentation of shift reports, transportation schedules, and group activities in the EMR is crucial, as is administering and submitting documentation for client Urine Analysis. The role also involves coordinating client lunch daily and serving as a role model to treatment staff, clients, employees, and guests through a positive, caring, and professional attitude. Maintaining confidentiality, complying with the dress code, and completing tasks accurately and on time are essential. The Care Coordinator must attend designated staff and in-service meetings and be able to maintain clear boundaries with clients and staff, setting limits and managing clients when direct supervision is needed. All documentation in EMR must be submitted within 24 hours.
