The Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. This role involves coordinating individualized services including orientation, transportation, doctor visits, and scheduling. The Care Coordinator ensures programs and services are implemented at the highest standards and clients receive the highest level of care. They are also responsible for client supervision, facilitating groups, conducting client orientations, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation, administering and submitting client Urine Analysis documentation, assisting with facility cleanliness, and coordinating client lunch daily. The role requires serving as a role model, maintaining confidentiality, complying with the dress code, completing tasks accurately and timely, and attending staff meetings. Additionally, the Care Coordinator must maintain clear boundaries, set limits, manage clients needing direct supervision, and ensure all EMR documentation is submitted within 24 hours.
