We are seeking a Health Home Care Manager who is passionate about removing barriers, closing care gaps, and uplifting the most vulnerable members of our community.

Requirements

• Bachelor’s Degree required (Social Work, Human Services, Psychology, Public Health, or a related field)
• One (1) year of related human services experience required in providing direct services to clients diagnosed with severe mental illness, HIV/AIDS or other disabilities, in order to link them to a broad range of services essential to successfully living in the community.
• You must have the ability and willingness to regularly travel, in some instances with clients in Agency vehicle to many locations using various modes of reliable and safe transportation
• Strong communication, organizational, and advocacy skills
• A deep sense of purpose and a commitment to serving vulnerable communities

Responsibilities

• Support clients as they move from hospital or rehab settings back into the community—ensuring continuity, safety, and support every step of the way.
• Conduct initial and ongoing assessments of clients to document strengths, needs, goals and resources.
• Schedule and coordinate timely follow-up with primary care and behavioral health providers.
• Identify missed appointments, medication lapses, or unaddressed needs—and take proactive steps to close the loop.
• Connect clients with resources such as housing, food security, transportation, and income/benefits support (SSI/SSD, SNAP, HEAP, etc).
• Work with a network of providers and support agencies to build individualized, person-centered care plans that truly make a difference.
• Provide face to face outreach, engagement, and service planning in the field including clients' homes, shelters, and hospitals
• Maintain documents, records, and other related reports in an organized, timely and accurate manner as per policy and procedure.
