About The Position

The Care Coordinator coordinates services for women and parents of infants who were screened for or referred to Healthy Start. This role involves providing education and support in childbirth, breastfeeding, women's health, and parenting to pregnant women and families with children aged 0-3. The coordinator will explain Healthy Start services, identify risk factors, and encourage consumer participation. They will also develop cooperative working relationships with primary health care providers, maintain updated progress notes, and manage a caseload of approximately 40+ consumers per month. Additionally, the role requires attending staff meetings and training, and potentially providing short-term psychosocial care if qualifications are met. Ensuring client records are accurate and updated, providing referral information for community resources, and documenting referrals and closures within the database system are key responsibilities. The coordinator must make required efforts to reach clients, follow telephone etiquette, and maintain a comprehensive understanding of maternal/child health issues. Generating necessary correspondence and maintaining established quantity and quality standards are also part of the role. A good working knowledge of the Well Family System and State of Florida Healthy Start Standards and Guidelines is essential.

Requirements

Graduation from an accredited four-year college with a degree human services, social sciences, social work, nursing, health education, health planning, healthcare administration, or related field is required.
2 years of public health/community development experience is required.
Must have reliable means of transportation.
CPR Certification, First Aid Certification and an annual Tuberculosis test.
Ability utilize Microsoft Office Software Suite.
Ability to provide an Affidavit of Good Moral Character.
Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.

Nice To Haves

Graduation from an accredited masters program with a degree in Social Work, Psychology or related field is preferred.
Bilingual is preferred.

Responsibilities

Responds to Healthy Start referrals in a timely manner consistent with Healthy Start requirements.
Provide education and support in childbirth, breastfeeding, women's health, and parenting to pregnant women and families with children aged 0-3.
Explain Healthy Start services to consumers, identify risk factors, and encourage consumers to participate.
Develop cooperative working relationships with primary health care providers.
Provide care coordination and referrals.
Maintain updated progress notes of services, referrals, and accomplishments in consumer files in a timely manner as outlined by Healthy Start Standards and Guidelines.
Ability to maintain a caseload of approximately 40+ consumers per month.
Attend staff meetings, training and in-service as assigned by the Program Director or required by the agency.
Ability to provide short-term psychosocial care, if qualifications are met.
Ensures that all clients records are accurate, current and updated on a regular basis.
Provide referral information to educate and improve clients knowledge and understanding of available community resources, as well as link clients to services that support the well-being of women during pregnancy and/or during childs infancy (e.g., WIC, food banks, housing, mental health service, etc.).
The referrals or information provided should be based on the risks identified through the prenatal or infant screen and/or the initial contact interview.
Intake coordinator may facilitate the referral process, to ensure client is connected.
Ensure referrals, as well as closures, have been appropriately linked and documented within the database system and agency data function.
Ensure required efforts are made when attempting to reach clients, as stated in the programs guidelines.
Follow appropriate telephone etiquette when answering incoming calls, when leaving messages (e.g., identify yourself and program, speak clearly when leaving messages, etc.) and or taking messages accurately by verifying callers information.
Develop and maintain a comprehensive understanding of maternal/child health issues that are deemed risk factors during pregnancy and early childhood.
Generate necessary correspondence in compliance with program guidelines of expected communications with clients and providers.
Responsible for maintaining established quantity and quality standards.
Professional work documentation and time management skills.
Develop and maintain good working knowledge of the Well Family System and State of Florida Healthy Start Standards and Guidelines.
Ensure referrals, as well as closures, have been appropriately linked and documented within the database system and agency data function.
